If you have both Medicare and Medicaid—and are dealing with special healthcare needs—you may be eligible for this D-SNP.
Optima Medicare Engage – Diabetes and Heart (HMO C-SNP)—NEW

Our Chronic Condition Special Needs Plan (C-SNP) supports members living with diabetes, congestive heart failure, or cardiovascular disease. Members receive personal attention from a care coordinator along with a team of high-quality doctors, specialty providers, and healthcare facilities to attend to their unique healthcare needs.
Optima Medicare's C-SNP is a Medicare Advantage plan offering more than Original Medicare, with prescription drug coverage and extra benefits like medical and non-medical1 transportation, a grocery allowance1, and an over-the-counter product allowance.
