Disclosure of Protected Health Information

Optima Behavioral Health is committed to protecting the privacy of a member's personal health information. No person, e.g. spouse, parent (of a child 14 years and over), agent/representative, employer, teacher or guardian, etc., can access a member's personal health information without the written permission of the member. A member can provide permission for access to any or all protected information by completing and submitting the appropriate form.
